The Letter
Illinois says it erased more than 1.4 billion dollars in medical debt for more than 694,000 people, with no application. The state spent under 10 million dollars. A letter is how people find out.
Illinois erased 1.4 billion dollars in medical debt. Nobody filled in a form.
On 15 October 2026, at the Cook County Health building, Governor JB Pritzker said the state's medical-debt programme had passed its own target. More than 1.4 billion dollars had been erased for more than 694,000 people, in all 102 counties. The original goal was 1 billion. The state spent less than 10 million dollars. The office put the return at more than 100 dollars of debt per state dollar.
The average, in the governor's release, is more than 1,200 dollars a person. In a remark reported two days later by The Center Square, Pritzker said the average recipient had nearly 1,600 dollars cancelled. Both sentences are his orbit. They are not the same figure. One person had more than 900,000 dollars erased. In 40 counties, the office said, more than 5 percent of residents received relief. Kankakee County was cited at nearly 22 percent.
People do not apply. The debt the programme buys is already at a collection agency, or about to be. A letter arrives and says the number is gone. Eligibility is generally a household income at or below 400 percent of the federal poverty level.
The Sun-Times added Cook County's earlier programme, which started with federal rescue money, and put the combined erasure at about 2.4 billion dollars for about 1.5 million people. County board president Toni Preckwinkle said the county piece would continue after the federal money ends. That is a commitment, not a statute we have read.
The announcement is eleven days old on the day we publish. It is in the feature window, not the last-24-hours window. It is a card for that reason, and because a letter is not a film when the same episode already has a door.
Erasing a bill a hospital has already sold is not the same as stopping the next bill. Studies of medical-debt cancellation have not all found a change in health or in financial stress. We are not citing a paper we have not opened. The limit is the mechanism: old debt, pennies on the dollar, a letter. Not a new health system.

Why This Matters
The administrative trick is the story. The state did not build a portal and wait. It bought debt that was already bad and told people by post. For the person who opens that letter, the number is real even if the next admission is not covered.
What We Don't Know Yet
Which average is the ledger average. Whether cancelled collection debt changes credit in practice, or the next bill. This programme does not cap hospital prices. The announcement is from 15 October, not from this morning.
